Excellent written and verbal communication skills. Ability to manage time management efficiently. Identifying and contacting community services regarding available resources as needed. Maintain documentation of outreach process for each client, including but not limited to progress notes, assessments and outreach attempts. Assess immediate needs, develop and implement action plan if necessary. Screen individuals to verify eligibility for services. 
Demonstrate thorough understanding of Health Homes program and services. Utilize various means of outreach to initiate contact with individuals referred for services.
